■ Procalcitonin (PCT) for Diagnosis of Sepsis and Differentiation of Serious Bacterial Infections

PCT is a non-hormonally active precursor of calcitonin, a glycoprotein composed of 116 amino acids which form a structure consisting of calcitonin, calcitonin gene-related peptide (CGRP), and fragments of N-terminal residues.

PCT levels rise rapidly within 2-4 hours following an infection, and peak at 12-48 hours. The concentration of PCT is positively correlated with the severity of the infection and returns to normal once the infection has resolved. This makes it highly valuable in the early diagnosis of severe bacterial infections, assessment of disease severity, prognosis, evaluation of anti-infective treatment effectiveness, and guidance on the use of antibiotics.

■ Interleukin-6 (IL-6) as an Aid in the Early Diagnosis of Acute Infection

IL-6 is an important cytokine that the innate immune system produces in response to injury and infection. It can promote the liver to produce acute-phase reactants and also stimulate bone marrow cells to generate more polymorphonuclear leukocytes.

In inflammatory responses, IL-6 is elevated earlier and the increased level persists longer than other cytokines. After bacterial infection, the level of IL-6 increases rapidly and reaches its peak at 2 hours, which is consistent with the severity of infection.
